    c) 受洗申請問卷下載 (中文)；Baptism Application Questionaire

若申請加入會籍，只需填寫申請表受洗/入會籍共用之申請表；若申請受洗，需填寫申請表＋受洗問卷。
If you are applying for church membership, please fill the baptism/membership application form (same form), if you are applying for baptism, please fill both the application form + the baptism questionaire.
